What is the physiological basis for a diagnosis of "dementia"?

Is there any? Has the research extended that far yet?

Or is a diagnosis based mostly or exclusively on clinical observations or tests of brain function?

I ask because I am 51 and suffering from chronic short term memory loss, and I am not satisfied with comments such as "Oh, everyone gets like that as they get older!"

What is the physiological basis for a diagnosis of "dementia"?
Generally on clinical observation, confusion, and age
