Can someone explain this to me?

I am reading a research paper on the risk of congestive heart failure in rheumatoid arthritis and there conclusion is: "Compared with persons without RA, patients with RA have twice the risk of developing CHF. This excess risk is not explained by traditional CV (cardiovascular) risk factors and/or clinical ischemic heart disease." Can someone explain that to me please if it is possible. Thank you so much.

If you controlled for all of things that are already known to cause CHF including ischemia (inadequate supply of oxygenated blood to the heart muscle typically causing changes in the muscle), people with RA would be still have an increased risk of developing CHF. This conclusion suggests that RA is an independent risk factor for CHF...that is, you could develop CHF if you have RA despite having no other reason to develop CHF.

Reply:People who have cardiovascular risk factors such as family history of premature cardiovascular disease, ethnic heritage, sedentary (no exercise), obesity (especially 'central' or abdominal), diabetes, hypertension, dyslipidemia (cholesterol), peripheral vascular disease and atherosclerosis have a higher risk of developing heart failure than someone without those risk factors and this increase in risk is known. People with Rheumatoid Arthritis have an increased risk of heart failure above and beyond that which would be predicted by their cardiovascular risk assessment and there is a theory behind this. Rheumatoid arthritis is an inflammatory disorder and inflammation is a powerful predictor of cardiovascular events such as stroke and heart attack. Reply:Simply argue that informed consent is rarely possible, and almost never in medical situations.





When I consent someone for an esophagectomy, for example, no one, but a skilled surgeon could understand the concept.





Not a chance. With an anatomy book, and a patient with an IQ of 120 or better, maybe, but it would take an hour. And they could never "understand" what Vtach is, or a chylothorax or any of the dozens of things I tell them every single day..





Just not possible, or realistic, a concept for the pseudo-intellectual elite to "discuss" in bioethics class.
Reply:informed consent? because the patient has the right to know whats going on with his body. the physician should be honest and clearly discuss the diagnosis and the prognosis of the condition. the treatment modalities should also be explained very well. however if there's any procedure that the patient has to undergo for the betterment of his condition, the dr. should then explain the pros and cons and ask if the patient ammends to that procedure. thats informed consent


Has anyone done training in social work?

I'm a psychologist working in private practice and work can can be a bit sketchy-sometimes busy sometimes quiet. I used to work in a child and mental health unit which I enjoyed, but to go back into this I would need to add a professional qualification either clinical psych or social work. Social work is 2 years rather than 3 and I can get funding. Has anyone done training in social work can you tell me what to expect and what are hjob prospects like in the 30K region, I have 5 yrs + experience in healthcare/education and 3 yrs management/budget/research which I would hope would mean I can get a job more at a middle management level in social work. Any advice??

Well I have my BSW with an LSW and in the area I live in I am having trouble fining work in my city but we are like that here with most professions anymore.





Social Work values often conflict with the values you learnd in psychology. I would suggest you select a program that will give you internship experience in the area you want to practice.





Social Work is usually not a high paying profession but with an MSW you should be able to make a good living
Reply:I'm assuming you are a MA level psychologist and not licensed, not eligible for an LPC or MFT license, or not involved in clinical practice? If you have been practicing under supervision with someone with a license you should investigate that option first.





The starting salary for an MSW is usually between 30-40K depending on what part of the country you are in (in the big cities it will be more in the 50-60K range. You don't indicate if you are looking for a clinical license or to do administrative. You might look at an advanced generalist program if you want to leave both options open. You can also choose a track in many programs that focus on one or the other. DA

related information: there are some reports that scleroderma is caused by or made worse by copper.





It is already known that copper causes sclerosis of the liver so that does seem to support its adverse action on scleroderma.

Has anyone had any bad experiences with the migraine drug Maxalt?

I was prescribed this yesterday and did a little research on the side effects. Some include cardiovascular effects which sound scary. I was just wondering if anyone has had these effects, to compare with the clinical study, and if so, the severity and frequency.

I have been on Maxalt for 3 yrs for migranes and I have not had any side effects. At times it felt like I could not live without it! I guess you will just need to weigh the benefits to the potential (and rare) risks. Good luck!
Reply:There is no medicine for H/A OR MIGRAINE. Not only these but for almost all painful diseases. Hence they become chronic.
